We accept the following payment options:

1) Payment in full prior to initial treatment appointment.  We offer a 5% courtesy if paid in full  by CASH or CHECK.

2) Half prior to initial treatment appointment and half at completion of treatment.

3) One third prior to initial treatment appointment, a third during mid treatment and  last third at completion appointment.

4) One forth prior to initial treatment appointment, divided into four payments, last at treatment completion.

5) CARE CREDIT: Full payment to our office. We pay the interest for 6 to 12 month options.                                 You will receive a monthly payment statement. There is not a courtesy with full payment due to payment of the interest for you.

                         Final payment is due at delivery of your restorations.

INSURANCE:

We require payment to be to our office for services rendered or, the above payment options. 

As a courtesy we will submit your insurance with all the necessary information to maximize your claim. You will be reimbursed by your insurance company.
